Title - What you're tracking

Purpose - Why are you tracking it?

Goals and measurements- How do you know you're achieving your purpose?

Objectives - Do these things on this schedule.

Baseline - What you are able to do when you start.
Log - What you wanted to do.  What you did.  When you did it.  How did what you did compare to what you wanted to do?  How did what you did this time compare to what you did last time?  How did you feel about it?

When the process occurs

Resolution/ Task Completion:

Steps to resolve the issue or complete the objective

State of knowledge (untested, tested, draft, archive, needs update)
